Background: The application of a tourniquet during surgery can effectively limit blood loss but may trigger ischemia-reperfusion injury in remote organs.
Aim: This study aimed to evaluate how reperfusion intervals during lower limb ischemia affect renal oxidative stress, indicated by malondialdehyde (MDA) levels, and histopathological changes.
Methods: Twenty-four male Wistar rats were randomized into three groups: Group C (n=8) underwent continuous 3-hour tourniquet application without reperfusion; Group R1 (n=8) received a 2-hour tourniquet followed by a 10-minute reperfusion period and an additional 1-hour tourniquet; and Group R2 (n=8) underwent a 2-hour tourniquet followed by a 20-minute reperfusion and a subsequent 1-hour tourniquet. Renal MDA concentrations were quantified using the thiobarbituric acid reactive substances (TBARS) method, and histopathological alterations were assessed through hematoxylin–eosin (HE) staining. Statistical analysis employed the Kruskal–Wallis test with Dunn’s post hoc comparison.
Results: Both reperfusion interval groups (R1 and R2) demonstrated significantly lower renal MDA levels compared with the continuous ischemia group (p=0.0026 and p=0.0016, respectively). Conversely, glomerular injury scores were significantly higher in Group R2 compared with Group C (p=0.003) and Group R1 (p=0.04). No significant differences were found in tubular injury among the groups (p=0.999).
Conclusion: Reperfusion intervals during prolonged tourniquet application reduce renal oxidative stress but do not prevent histopathological damage.
